A Retrospective Audit of Post Discharge Outcome for Patients Supported by ‘Acute Coronary Syndrome Support Network’ to Remote Communities in the Northern Territory of Australia

Abstract
Background: Australians living in remote areas have higher rates of hospitalisation and mortality due to coronary heart disease (CHD) (AIHW 2014). 81% of ATSI Australians within the Northern Territory live in remote and very remote areas (ABS Census 2011). The NT Cardiac 24/7 ACS support network was established in late 2013 to provide early ECG interpretation and direct specialist guidance for patients with suspected ACS in remote communities. Aim: To assess the standard care for these confirmed ACS patients based on National ACS care standards which are important for secondary prevention. Method: A retrospective review undertaken for 52 patients who had confirmed diagnosis of ACS and were supported through network. Clinical information was obtained from various data sources which included medical record and electronics records from acute care, primary care and private systems. Results: 52 patients (out of the first 100 consecutive patients supported via ACS network) were confirmed to have ACS. 31 (60%) received invasive coronary angiogram, 43 (84%) were offered an individual plan, 36 (69%) were discharged on dual antiplatelet therapy, 36 (69%) were discharged on lipid lowing medications, 26 (50%) were discharged with a rehabilitation referral and 38 (73%) had discharge summaries send to the GP or health care provider within 48 hours of discharge. Conclusion: Despite the increased access in the early specialist support for remote ACS patients in the NT, there are still significant gaps in the post discharge ACS care standards and needs to be addressed through system of care.
