This implementation plan provides an outline of strategies and actions for chronic conditions prevention and management from 2010 to 2016 in government and non-government health organisations, workplaces and communities throughout the Northern Territory. The plan will be updated every three years throughout the life of the strategy. The strategy is a part of Dept. of Health Healthy Territory Initiative at http://www.healthynt.nt.gov.au.
