This study aims to compare short and long-term outcomes of 1470 preterm infants born between 23+0 and 28+6 weeks gestation who have respiratory carein the delivery room with A) initial FiO2 0. versus B) initial FiO2 0.3, followed by common SpO2 targeting until admission to NICU. Just prior to delivery, babies are randomlly allocated to the groups above and at delivery, initial FiO2 is set to 0.6 or 0.3 as randomised. After 3 minutes from birth or at the clinicians discretion, FiO2 is adjusted in increments of 0.1 - 0.2 every seconds to target SpO2 as per NRP guidelines. other care follows local protocol. The primary composite outcome is death or survival free from major brain injury. The secondary outcomes are all-cause mortality and major brain injury.
