Description
The aim of the strategy is: ‘To improve the health and wellbeing of all Territorians by reducing the incidence and impact of chronic conditions.’ This strategy is a continuation of the past work of health leaders and reinforces the need for strategic and integrated approaches to improve health outcomes in our community. (2) The strategy prioritises population-wide approaches to reduce risk factors for chronic conditions, particularly for ‘at-risk’ groups. System-wide approaches aim to improve health outcomes by strengthening service delivery.
