Since the introduction of the PBS, the rate of treatment uptake for Hepatitis C in Australia has declined substantially. A concern is that this decline in treatment is due to poor retention in care between first point of contact and administration of treatment. The mergence of highly accurate, non-invasive point of care (POC) testing for both HCV antibodies and HCV RNA may overcome some of the major barriers. This study will use a cross over randomised control trial with four intervention conditions, as follows: ARM A: Rrapid HCV antibody (QraQuickTMOrasure) testing folowed by standard community-based treatment. ARM B: HCV rapid antibody (QraQuickTM OraSure) and rapid RNA test (GeneXpertTM, Cepheid) followed by standard community-based treatment. ARM C: rapid HCV antibody (QraQuick TM OraSure) testing followed by same-day HCV treatment initiation model. ARM D: Strandard of Care HCV management requring multiple diagnostic and prescribing visits using laboratory testing and standard prescribing rules (current standard of care) . Each site will undergo a control intervention (D) and one study intervention (A-C, randomly allocated); 3 month periods. Sites will include clinics in metropolitan and rural settings with high caseloads of PWID.
