| Abstract |
The NT Aboriginal Health Plan 2015-2018 (this Plan) sets out the strategic directions that will guide our actions to improve the health and wellbeing of Aboriginal1 Territorians. We will do this by working in partnership with the Aboriginal community controlled health sector, other government and non-government agencies, and with Aboriginal people and communities to address the social determinants of health and achieve the NT Health Strategic Plan 2014-17 vision of ‘Healthy Territorians living in healthy communities’.
